We welcome new patients! To make the most of your visit, please arrive 15 minutes early so you receive your full treatment time. All appointments are secured with pre-payment.

We require a 24-hour notice if you need to cancel or reschedule your appointment.Â
If you are late for your appointment time, your treatment will be shortened so we finish on time for the next patient. The fee for the treatment will remain the same.
We recommend booking your next appointment before you leave to ensure your preferred date and time.
Gift certificates make a great gift! They are:
Valid for one year from the date of purchase.
Non-refundable.
Treated as cash (we cannot replace lost or stolen certificates).
We require 24-hour notice in the event of cancellation. If you are more than 10 minutes late, you will give up your spot and lose that treatment. No exceptions.
Please come ready for your appointments. Please wear comfortable, casual, quick-to-remove clothing. For some services you will be asked to wear a gown while for other services you may remain clothed. This is not the time to wear your newer, better clothing.

The right to decline contact with anyone not directly involved in your care.
Private phone use and a private setting for exams or conversations.
A same-sex staff member present during certain exams if requested.
To only be disrobed as needed for your care.
To provide written consent for any filming, recording, or release of your medical information beyond legal requirements.
To help us provide the best possible care, we ask that you:
Provide complete and accurate medical information, including:
Details about your current condition or illness.
Past medical history (illnesses, surgeries, hospitalizations, or changes in health).
Any specific symptoms, pain, or concerns.
All medications (prescription, over-the-counter, and herbal), including dose and frequency.
Any allergies or adverse reactions (medications, food, environmental, latex).
Any changes in your medical condition.
Keep your appointments.
Ask questions if you don’t understand your treatment plan or your role in care.
Make informed decisions about your care and follow the agreed-upon treatment plan before, during, and after surgery.
Arrange for someone to:
Drive you to and from your surgical appointment.
Stay with you for 24 hours after surgery.
Follow Rejuuuuve4You’s policies to support a safe, respectful environment, including:
Respecting the rights of other patients and staff.
Following our smoke-free campus policy.
Knowing the name of your surgeon.
Provide accurate payment information and fulfill your financial obligations in a timely manner.
Take responsibility for your care if you refuse treatment or don’t follow medical advice.
Notify us of any issues or complications after your surgery.
Inform your provider of any advance directives (living will, medical power of attorney, etc.).
